cotisation annuelle obligatoire et \u00eatre conscient que toute fausse d\u00e9claration expose \u00e0 des poursuites p\u00e9nales ; les dossiers incomplets sont refus\u00e9s.<\/li>\n\n\n\n<li><strong>Formulaire de demande<\/strong><br>T\u00e9l\u00e9charger le formulaire, le compl\u00e9ter, le dater, le signer puis le scanner.<\/li>\n\n\n\n<li><strong>Attestation de t\u00e9moin<\/strong><br>T\u00e9l\u00e9charger l\u2019attestation obligatoire, l\u2019imprimer, la dater, la signer et la scanner au format PDF.<\/li>\n\n\n\n<li><strong>Pi\u00e8ces justificatives<\/strong><br>Fournir une pi\u00e8ce d\u2019identit\u00e9 valide (carte d\u2019identit\u00e9 ou passeport) et un justificatif de domicile de moins de deux mois avec filigrane.<\/li>\n\n\n\n<li><strong>D\u00e9p\u00f4t de plainte<\/strong><br>Joindre le d\u00e9p\u00f4t de plainte avec constitution de partie civile, indispensable pour permettre le suivi du dossier par les avocats conseils de la FFPAnimale.<\/li>\n\n\n\n<li><strong>Envoi du dossier <\/strong>: (au format pdj) sur signalements@ffpanimale.fr
